Directions:

5 Simple Steps

1. Fill out the Form. (Answer all the questions you want. (Remember the more you answer the better!)
2. Place the form in the vial. (Include copies of SIGNED Do Not Resuscitate Orders (DNR), Living Wills, copies of past EKG’s, Picture of yourself, and anything else you think may be helpful for treatment.)
3. Place the vial in your refrigerator, preferably on door.
4. Place magnetic decal on your refrigerator door.
5. Tell your friends and family about the Vial of Life program and tell them to contact the Florence Twp Fire Department to get their own.
